Housing Stablization Services (HSS)
HSS is a dedicated Medical Assistance benefit designed to assist individuals with disabilities and seniors in finding and maintaining stable housing. Our comprehensive suite of services is tailored to address the unique needs of each individual, promoting independence and ensuring a safe, secure living environment. At OCS, we are committed to providing compassionate, individualized support to enhance housing stability and improve overall well-being.
Eligibility Critarea
To access our Housing Stabilization Services, individuals must meet the following eligibility criteria :
- Be on Medical Assistance (MA)
- Be 18 years or older
- Have a disability or disabling condition

Require assistance in at least one of the following areas due to a disability :
- Communication
- Mobility
- Decision Making
- Managing challenging behaviors
- Have experienced housing instability, including homelessness, transitioning from an institution, or being at risk of homelessness or institutionalization.
Housing Consultation
Housing Consultation is a comprehensive service tailored to individuals without Medical Assistance (MA) case management services. The aim is to address the unique needs of each client through a personalized approach. OCS’ Housing Consultation serves as a one-stop-shop, for developing a Housing Focused- Person Centered Plan (HF-PCP) and Professional Statement of Need (PSN), guiding individuals through the complexities of housing-related processes, promoting autonomy, and fostering a client-focused approach to housing stability.
OCS Housing Consultation service include the following key components :

HSS Eligibility Documentation Assistance :
- Assist individuals in gathering and preparing the necessary documentation for HSS eligibility. This involves ensuring that all required paperwork is completed accurately and efficiently.

Development of Housing Focused Person-Centered Plan (HF-PCP) :
- Works closely with clients to develop a Housing Focused Person-Centered Plan, customized to the individual’s circumstances. This plan is created based on thorough assessments to ensure it aligns with the specific needs and preferences related to housing.

Professional Statement of Need (PSN) :
- The service includes the creation of a Professional Statement of Need, providing a comprehensive overview of the client’s housing situation, disabling condition and need for HSS. This document serves as a crucial tool in determining eligibility and approval through the Department of Human Services (DHS).

Identification of Strengths, Needs, and Preferences in Housing :
- Conducts a detailed analysis to identify the foundation for crafting a plan that addresses the client’s unique circumstances.

Coordination with Existing Service Providers :
- This coordination offers a seamless integration of services and aims to enhance the overall support provided to the individual. Ensuring a holistic approach to their housing needs.

Privacy Rights and Appeal Information :

Annual Updates to Person-Centered Plan :
- The HF-PCP is an annual plan, in which we will provide support ensuring that the plan remains relevant and effective over time.

Our goal is to facilitate a smooth, well-planned transition by connecting individuals with essential services like medical, social, educational, housing and resources tailored to their unique needs and circumstances.
